That Matter

"Conversations That Matter" is a student project during which we developed a toolkit of 54 prompt cards to help terminally ill patients, caregivers, and healthcare professionals navigate end-of-life conversations. It covers topics like health, digital legacy, and memorial wishes, offering 
a practical and empathetic approach to these sensitive discussions. The toolkit is available in both physical and digital formats.
